If your pet has high blood pressure, your veterinarian may prescribe telmisartan. It is used to treat a variety of conditions including high blood pressure, proteinuria, the inappropriate protein loss by urination in dogs and cats. It is also used to treat some forms of kidney and heart disease, and to reduce side effects from other blood pressure medications. High blood pressure or hypertension adds work to the arteries and heart, therefore it’s important to treat it to avoid long-term damage.
Telmisartan is an angiotensin-II receptor blocker that works by blocking substances that cause blood vessels to tighten in the body. It's a biochemical formed to maintain and relax the blood vessels, therefore lowering blood pressure, and increasing the supply of oxygen and blood to the heart. It can also alter the way blood flows through the nephrons in the kidneys.

Uses

Telmisartan compounded oral liquid is given by mouth in liquid form once daily with or without food. This medication should be given just as prescribed by your veterinarian. It must be measured carefully, and it's best to use the provided syringe to ensure you are able to give the proper dosage required. After dosing your pet, rinse and dry the syringe, and screw the cap tightly onto the bottle.
If you have difficulty giving it to your pet, contact your veterinarian for some helpful tips. We recommend washing your hands with soap and water after handling any medication.

Possible Side Effects

Telmisartan Compounded Oral Liquid is generally safe, however, the most common side effects include vomiting, dehydration, diarrhea, weight loss, lack of appetite, and lower energy levels. More severe adverse effects may be collapse or extremely low energy levels. Contact your veterinarian immediately if these side effects continue or become more severe.

Drug & Food Interactions

If your pet is on any of the following medications you should use telmisartan with caution:

  • Ace inhibitors - (e.g. benazepril, enalapril): When combined can increase risk for adverse effects.
  • Aspirin - When combined can reduce the antihypertensive effect, and increase risk for renal impairment.
  • Baclofen - When combined may have additive blood pressure lower effects.
  • Corticosteroids - (e.g. betamethasone, dexamethasone): When combined may decrease the antihypertensive effects of ARBs.
  • Dexmedetomidine - When combined may result in additive effects on blood pressure and heart rate.
  • Digoxin - When combined may increase digoxin levels.
  • Insulin - When combined may increase risk of hypoglycemia.
  • Low molecular weight heparins - (e.g. dalteparin, enoxaparin): When combined may increase risk of hyperkalemia.
  • Nsaids - (e.g. carprofen, meloxicam): When combined can reduce antihypertensive effects of telmisartan and increased risk for renal impairment.
  • Potassium preparations, potassium-sparing diuretics - (e.g. spironolactone): When combined can increase the risk for hyperkalemia.
  • Yohimbine - When combined may decrease the efficacy of ARBs.

The following medications when combined with telmisartan may increase the risk of hypotension; benzodiazepines, buspirone, cabergoline, diazoxide, dichlorphenamide, diphenhydramine, doxepin, and opioids.

Precautions

Telmisartan should not be given to pets that are pregnant, dehydrated, have low blood pressure, or are hypersensitive or allergic to it. If your pet is electrolyte depleted, they should not take this medication until their blood volume and electrolytes have been restored.
Telmisartan should be used with caution in pets that have a hepatic impairment, a lack of liver functions.

Monitoring

Your veterinarian will likely monitor your pet for blood pressure, kidney function, vomiting, diarrhoea, or weight loss.
Depending on the reason for the treatment your vet may also check the urine protein, renal function, volume status, and liver enzymes.
Red blood cell counts should also be monitored every 3-6 months.
